Remote Patient Monitoring (RPM)
Medicine has changed significantly over the past decade. Doctors and other health care providers are now able to assist you with the remote management of your preventive health right in your home. A nurse or other medical provider can visit you in your home to set-up electronic devices to check your temperature, blood pressure, heart rate, and other important parameters of your health. The information collected by the devices that are placed in your home is transmitted to the provider and to your primary care doctor wherever they are located. We can review the information from the devices that are left in your home, and help you make informed decisions about your health. Remote Patient Monitoring (RPM) is provided in conjunction with visiting nursing services and peer-reviewed electronics provider organizations.
Chronic Care Monitoring (CCM)
Chronic Care Monitoring (CCM) is essentially the same as Remote Patient Monitoring (RPM), but is available for patients who have chronic medical conditions like diabetes or heart failure. The information collected by the devices that are placed in your home is transmitted to the provider and to your primary care doctor wherever they are located. This CCM feature is especially convenient for patients that are bedridden or handicapped. This service is provided in conjunction with visiting nursing services and peer-reviewed electronics providers.
Nutrition & Weight Control Counseling
We understand that weight control is an important part of managing your preventive health. Our nutrition and weight control counselors are available to assist you in managing your weight and your nutrition whether you are on a vegan diet, a vegetarian diet, or a regular diet. Individual and group counseling sessions can be arranged in these areas.
