After we have reviewed your completed questionnaire and verified your enrollment, we will email you a unique computer identification that you can use to link to your Personalized Preventive-health Guidelines (PPG). The Guidelines are intended to assist you in managing some of the key areas of your preventive health. Note that preventive health guidelines may vary by age, gender, country and by location.
Your answers to this HealthCheck™ questionnaire are confidential and are anonymized (not associated with your specific name). The information is used for educational purposes, to personalize the guidelines for managing your preventive health and to identify preventive health trends in your age group. We do not sell your information. This HealthChek™ program is HIPAA compliant and is focused only on your preventive health.
